In booking this appointment, I understand that these online consultation services and recommendations are solely based on the information provided by me. And, in the absence of a physical evaluation, our care provider may not be aware of certain facts that may limit or affect their assessment, diagnosis and/or recommended treatment.

I understand that I will be asked to provide my credit card information to reserve my appointment time, and that appointment fees are non-refundable. In requesting this appointment, I understand that while services offered are considered clinical care, which may be covered by health insurance providers, MPP cannot guarantee that my insurer will reimburse me.

I agree to pay in full for any and all services rendered by MPP’s care providers. I understand that it is my responsibility to submit my receipt for this service to my insurance provider for reimbursement.
